Your health and quality of life matter to Great River Health! Did you know that only 20% of your health is impacted by genetics and healthcare? That means that 80% of your health is influenced and impacted by environmental factors. These “social determinants of health” are conditions you encounter in your environment including where you were born, live, learn, work and play. These things impact health beyond genetics and lifestyle choices.

If you encounter any of the five conditions listed below, we have identified regional resources that can potentially help your situation. Things like safe housing, transportation, education, job opportunities, access to nutritious foods and more can make a significant difference in people’s health and quality of life. See the descriptions below for more information followed by resources to contact for help.

The Social Determinants of Health information is from Healthy People 2030, U.S. Department of Health and Human Services, Office of Disease Prevention and Health Promotion. For more information, visit: https://health.gov/healthypeople/objectives-and-data/social-determinants-health
