Kathleen Canella Family Scholarship Application

These scholarships are awarded to students pursuing higher education in a medical field (excluding physicians). The Kathleen Canella Family Scholarship(s) shall be awarded on the basis of financial need and scholastic history.

To be eligible, you must plan to continue your education beyond high school at an accredited community college, technical institute or four-year college or university providing medical education.

The Kathleen Canella Family Scholarship will be awarded to students graduating from any high school within the Southeast Iowa Regional Medical Center market area. Non-traditional students will also be considered.

Application Process

A complete application includes the six documents below:

1. Application Form
2. Letter stating why I believe I should be a recipient
3. Letter of support from a personal contact (non-relation)
4. Letter of support from a professional contact
5. EFC from FASFA
6. High school transcript for the past 7 semesters (If applicable)
