MyGreatRiver Patient Portal
A patient portal is a secure website that gives patients access to their
personal health information. You can:
- Manage appointments
- Communicate with your health care provider
- Get information about your health conditions
- Review your medical history
You can manage multiple family members’ health information from one
portal. Adults must provide written consent for another person to manage
their medical information.
The portal is for routine management of your health care. If you are having
a medical emergency, call 911 or your provider’s office immediately.
How to sign up for the patient portal
For your protection, you must sign up for MyGreatRiver in person at:
- Any Great River Health System patient registration area
- Health Information Management
- Patient Financial Services-Patient Billing
Health App Information
Great River Health System is planning to offer a resource for securely
connecting some of the health management apps you use – like fitness
and dietary trackers – to your health record. This new process will
enable you and your health care team to make informed choices using your
health and wellness data.
If you are interested, please send us names of your preferred apps below.
After we receive your request, we will work with app vendors to determine
if they meet the technical requirements for establishing a secure connection.

can access only this information about their children ages 12 to 17 through
the patient portal:
The change in access will become effective on the child’s 12th birthday.
Parents and legal guardians can get complete medical records for their
children through age 17 in the Health Information Management Department.
